2022-2023 Girls Registration
This form is to be completed by all participants registering for GIRLS hockey with Mitchell Minor Hockey. Sport administers in Ontario have been mandated to comply with certain participant/registrant requirements. Government legislation requires participants of sport to acknowledge specific mandatory waivers and forms to ensure that all players comply in completing the appropriate waivers to participate. Players, Parents and Officials are to carefully read all content within the form of this registration package.

Mitchell Minor Hockey collects your personal information so that we may fulfill our responsibilities of providing hockey programs in a cost efficient manner. Your personal information will be used by MMH for its programs. MMH will not sell your personal information to anyone.

Registrants and their families will abide by the MMH Constitution, By-Laws and Code of Conduct. 


Mitchell Minor Hockey applies a penalty of $25 for all NSF cheques.  Your child will NOT be permitted to participate in any MMH programs until full payment is made.

I give permission to have my child's picture taken and posted on the MMH website.

I understand that Each MMH family is required to complete 5 VOLUNTEER BOND HOURS. It is mandatory 3 credits be completed helping at MMH tournaments, unless you are fulfilling a full service hours position such as parent rep or bench staff. I will submit a post dated bond cheque to my parent rep for $750 (dated April 1, 2023)

Acknowledgement of Review

The above named Participant and Parent/Guardian (if applicable) has/have reviewed the Mitchell Minor Hockey Code of Conduct (By-Law 1) and commit to the standard of behaviour that is expected of all Mitchell Minor Hockey Association ( MMHA) members, including players, coaches, parents, directors, and volunteers as outlined in the above link.

RESPECT IN SPORT : OMHA CODE OF CONDUCT

OMHA CODE OF CONDUCT This Code of Conduct identifies the standard of behaviour which is expected of all Ontario Minor Hockey Association (“OMHA”) members and participants, including but not limited to all players, guardians, parents, coaches, officials, volunteers, directors, officers, committee members, convenors, team managers, trainers and administrators involved in OMHA activities and events. The OMHA is committed to providing an environment in which all individuals are treated with respect. Members and participants of the OMHA shall conduct themselves at all times in a manner consistent with the values of the OMHA which include fairness, integrity and mutual respect. During the course of all OMHA activities and events, members shall avoid behavior which brings the OMHA or the sport of hockey into disrepute, including but not limited to abusive use of alcohol, use of non-medical drugs and use of alcohol by minors. OMHA members and participants shall at all times adhere to the OMHA operational policies and procedures, to rules and regulations governing OMHA events and activities, and to rules and regulations governing any competitions in which the member participates on behalf of the OMHA. Members and participants of the OMHA shall not engage in any activity or behaviour which interferes with a competition or with any player or team's preparation for a competition, or which endangers the safety of others. Members of the OMHA shall refrain from comments or behaviours, which are disrespectful, offensive, abusive, racist or sexist. In particular, behaviour, which constitutes harassment, abuse or bullying, will not be tolerated. Failure to comply with this Code of Conduct may result in disciplinary action, including but not limited to, the loss or suspension of certain or all privileges connected with the respective Member Association in the OMHA including the opportunity to participate in the OMHA and its’ Member Association activities and events, both present and future.

Completion of the Respect in Sport “Parent Program” for at least one parent/guardian of each family of every registered player (under the age of 18) shall be a condition of their eligibility for participation.

INFORMED CONSENT- WAIVER OF LIABILITY AGREEMENT

 

WARNING! Please read carefully! By signing this document, you will assume certain risks and responsibilities

 By signing you are stating that you have read and agree to the following:

(COVID-19) The COVID-19 disease has been declared a worldwide pandemic by the World Health Organization and COVID-19 is extremely contagious. Mitchell Minor Hockey (MMH) has put in place preventative measures to reduce the spread of COVID-19; however, MMH cannot guarantee that the Participant will not become infected with COVID-19. Further, participating in the Activities could increase the Participant’s risk of contracting COVID-19

 The Participant is participating voluntarily in the Activities. In consideration of that participation, the Parties hereby acknowledge that they are aware of the risks, dangers and hazards and may be exposed to such risks, dangers and hazards. The Parties understand that MMH may fail to safeguard or protect the Participant from the risks, dangers and hazards of the Activities related to Covid 19 as listed below:

(COVID-19) That COVID-19 is contagious in nature and the Participant may be exposed to, or infected by, COVID-19 and such exposure may result in personal injury, illness, permanent disability, or death

Release of Liability and Disclaimer

In consideration of MMH allowing me to participate, I agree:

·         That the sole responsibility for my safety remains with me

·         To ASSUME all risks arising out of, associated with or related to my participation

·         That I am not relying on any oral or written statements made by MMH or its agents, whether in a brochure or advertisement or in individual conversations, to agree to participate in the Activities

·         To WAIVE any and all claims that I may have now or in the future against MMH

·         To freely ACCEPT AND FULLY ASSUME all such risks and possibility of personal injury, death, property damage, expense and related loss, including loss of income, resulting from my participation in the Activities

·         To RELEASE MMH from any action related to my becoming exposed to or infected by COVID-19 as a result of, or from, any action, omission or negligence of myself or others, including but not limited to the Organization

    By signing below you are stating that you have read the above information and agree to be bound by the following Acknowledgement:

I am participating voluntarily in the Activities. In consideration of my participation, I hereby acknowledge that I am aware of the risks, dangers and hazards associated with or related to the Activities. I understand that the MMH may fail to safeguard or protect me from the risks, dangers and hazards specifically related to Covid 19.
