Proof of Vaccination
As per the recent Provincial, Municipal, OMHA and OWHA vaccination policies released, the following cut off dates will be mandated for players born PRIOR to 2010 (including 14 days post vaccination):

Girls – October 17
Boys – October 31

MMH is committed to making changes to our programming to allow the safe progression of hockey within our association. If you have a child currently registered in MMH who was born PRIOR to 2010 and you DO NOT plan on having your player fully vaccinated by the dates listed above please see the information below:

  1. Refunds will be issued in a timely manner per our 2021/2022 refund policy, however you will not be able to rejoin programming for the duration of the 2021-2022 season under the current guidelines. 
  2. On November 1, 2021, any affected person not fully vaccinated (i.e. including the 14-day period after receiving their completed dose) or who have not been granted an accommodation from the OMHA or OWHA in accordance with the Ontario Human Rights Code (OHRC) will no longer be eligible to participate in any sanctioned activities until such time as they have presented satisfactory evidence that they have been fully vaccinated or been granted an accommodation. 

For players born prior to 2010, you will be required to have both vaccinations by the outlined dates above.  If you need to discuss this further please contact Jaclyn Dietz at 519-301-4653 or by email at [email protected].

All players will be asked to present to MMH evidence satisfactory to the OMHA and/or OWHA demonstrating that they have received the completed series of an accepted COVID-19 vaccine.

Please note, if your child has begun their covid vaccination series and have only received one dose, they CAN attend tryouts knowing that they will need two doses by the dates outlined above.

MMH will be providing further details regarding the collection of acceptable documents serving as evidence of COVID-19 vaccination.


As of September 22, all entrants to MMH facilities (Mitchell and Monkton) will be required to be fully vaccinated.  Youth who are participating in hockey who are under age 18 will be allowed into the facility without showing proof of vaccination however, this will change once the OMHA and OWHA protocols are enforced on October 22 and October 31. Vaccine passports will be monitored by both facilities. Only the main doors at Mitchell arena will be used for all players and spectators.

Please note MMH does not have the ability to grant accommodations to the vaccination policies referenced.
